1. Acknowledgment of Risk
I, the undersigned participant or legal guardian of the participant, hereby acknowledge that participation in soccer training, games, and related activities involves inherent risks, including but not limited to, physical injury, illness, or damage to personal property. I understand that these risks can occur during drills, games, or any other activities related to the soccer program.
2. Assumption of Risk
I voluntarily choose to participate or allow my child to participate in the soccer program, fully understanding and accepting all risks, whether known or unknown, associated with such participation. I agree that my involvement is at my own risk and that of the participant.
3. Release of Liability
In consideration of being allowed to participate in the soccer program, I hereby release, discharge, and hold harmless Phoenix Soccer Academy, its directors, coaches, staff, volunteers, and any associated entities or organizations from any and all liability, claims, demands, actions, or causes of action arising out of or related to any injury, loss, or damage that may occur as a result of participation in the soccer program.
4. Medical Consent
In the event of an injury or illness requiring medical attention, I authorize Phoenix Soccer Academy and its representatives to seek and obtain necessary medical treatment for the participant. I understand that I am responsible for any and all medical expenses incurred as a result of any injury or illness during participation in the soccer program.
5. Insurance
I acknowledge that I am responsible for providing my own health and accident insurance coverage for the participant. I understand that Phoenix Soccer Academy does not provide any insurance coverage for injuries or illnesses that may occur while participating in the soccer program.
6. Code of Conduct
I agree that the participant will adhere to the rules and regulations set forth by Phoenix Soccer Academy and demonstrate good sportsmanship at all times. I understand that failure to comply with the rules and conduct standards may result in suspension or expulsion from the program without any refund of fees.
7. Media Release
I grant Phoenix Soccer Academy permission to use photographs, videos, and any other media that may include the participant for promotional and educational purposes, without any form of compensation.
8. Agreement to Terms
By signing this waiver, I confirm that I have read, understood, and agree to all the terms and conditions outlined above. I am aware that this waiver is a binding legal document and agree that it is intended to be as broad and inclusive as permitted by law.
